What release do you use? I had similar problems and the reasons has been "Unified Rendering". You can check if you have those kind of problems using report WDG_MAINTAIN_UR_MIMES. The values under CLUR_NW7=>VERSION_PROPERTIES and Deployed version.properties should be identical.
